INFORMATION FOR FOX LEA FARM SUMMER SHOW SERIES:

All horses will be required to provide :

1. Proof of current Negative Coggins

2. A 14 Day Statement of Health, certificate of veterinary inspection or a health

certificate.

3. Trainer/Owner Health Declaration Form- SEE FORM BELOW
